Course Overview

Health records were first used to document professional encounters with a patient over time. Later, more systematic documentation was needed to compare patient healthcare and outcomes. Electronic health records are intended to not only collect that systematic information, but also integrate the documentation across various professions. With progressively complex healthcare teams and healthcare decisions, new models of documentation are needed to support coordinated collaborative interprofessional practice. The goal of this course is to describe how members of an interprofessional (IP) team can systematically document care plans and outcomes to best inform evidence-based practice.
The content in this course is applicable to occupational therapy, speech-language pathology, audiology, and social work professionals in acute care facilities.
