In such organizations, being on the alert for how patients can be protected from harm is part of everyday work life. Although high-reliability organizations have few adverse events, they still continually explore risks and ways patient safety may be compromised. They view a close call or near miss as a signal to examine conditions closely in which a patient could have been injured. Organizations that value a culture of safety expect and reward high-quality care. All healthcare workers directly involved in patient care as frontline managers and must be involved in the prevention of medical errors.
