Summary:

Health records were first used to document professional encounters with a patient over time. Later, more systematic documentation was needed to compare patient healthcare and outcomes. Electronic health records are intended to not only collect that systematic information, but also integrate the documentation across various professions. With progressively complex healthcare teams and healthcare decisions, new models of documentation are needed to support coordinated collaborative interprofessional practice. 

Objectives:

The goal of this course is to describe how members of an interprofessional (IP) team can systematically document care plans and outcomes to best inform evidence-based practice.
 
The content in this course is applicable to occupational therapy, speech-language pathology, audiology, and social work professionals in acute care facilities.
After taking this course, you should be able to:
  • Describe the history and purpose of health records.
  • Explain how an IP team can use meaningful electronic health records (EHRs) to communicate effectively, influencing improved health outcomes.
  • Determine patient roles and public policies regarding the use of EHRs.
  • Identify how systematic documentation can be used to inform health outcomes.
  • Describe the role of health information management (HIM) professionals as members of IP care teams.
 

 
Accreditation Information
This course is intended for an interprofessional audience, including occupational therapists,  social workers, and speech-language pathologists.
 
Relias LLC, provider # 1273, is approved as a provider for social work continuing education by the Association of Social Work Boards (ASWB) www.aswb.org, through the Approved Continuing Education (ACE) program. Relias LLC maintains responsibility for this course. ACE provider approval period: 02/10/2018 - 02/10/2021. Social workers should contact their regulatory board to determine course approval. Social workers participating in this course will receive 1.0 general continuing education clock hours.
 
Relias LLC is also accredited by the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling (provider # 50-1489).
 
Relias, LLC, SW CPE is recognized by the New York State Education Department's State Board for Social Work as an approved provider of continuing education for licensed social workers #0009. This approval is for a three-year term beginning January 1, 2018 and ending December 31, 2020.

After reading the presentation, complete the course test online. With a score of 75% or better and completing the evaluation, you may immediately print your CE certificate of completion or save a pdf of it for your records. You may also request a copy to be sent to you by mail. If your score is less than the passing score of 75%, you may retake the test. In the event of loss, there is no charge for a replacement copy of a CE certificate.

When a participant, either orally or in written format, files a grievance, a member of the Provider Unit Planning Committee will evaluate the nature of the complaint. If it is meritorious, a member of the Provider Unit Planning Committee will instruct the staff to implement any necessary changes in the course. The complainant will receive a response from a member of the Provider Unit Planning Committee or designated staff within 5 working days.
 
If the results of the grievance process are unsatisfactory, New Jersey social workers have the option to contact the approving entity, with an option to appeal to their licensing board.
