Objectives:

The goal of this program is to provide speech-language pathologists and audiologists with information about laws and standards governing documentation, legal basics for appropriate documentation, and strategies for documenting changes in a patient’s condition. After studying the information presented here, you will be able to:
  • Identify the elements that speech-language pathologists and audiologists should include in their documentation
  • Provide the legal definition of negligence
  • Describe four characteristics of legally credible charting and two charting practices that can lead to legal problems
