Use of the interprofessional team approach has led to improved outcomes in some patient populations and disease states. Many different models exist and all can be adapted to fit specific population needs. As healthcare reimbursement continues to evolve, team-based approaches may be warranted to optimize patient care.


The goal of this continuing education module is to provide physicians, nurses, advanced practitioners, pharmacists, dietitians, speech-language pathologists, audiologists, respiratory therapists, and other providers with an introduction into the structure and function of the team-based care model. It serves to outline the core principles and values of healthcare teams and their value in the treatment of chronic disease. This module will address roles and responsibilities, interprofessional communication, and interprofessional teamwork. After studying the information presented here, you will be able to:
  • Identify the core principles and values of an effective healthcare team
  • Give an example of team-based care in your field of practice
  • Discuss or explain how evolving state legislation may impact the healthcare team
