More than 133 million people in the U.S. (nearly one in every two adults) live with chronic diseases. Chronic diseases are responsible for seven of 10 deaths in the U.S. More than 75% of the U.S. healthcare spending is on the care of chronic conditions. Six chronic conditions (arthritis, cancer, cardiovascular disease and stroke, diabetes, obesity-related disabilities, and respiratory diseases) account for more than $750 billion in healthcare costs annually. Many patients experience a poor quality of life, sorting through confusing and often conflicting information about medications and treatments. Disease management fills the gap in patient information needs, and helps educate and support patients in caring for their chronic disease.
The purpose of this disease management program is to provide respiratory therapists with information about disease management and how standardized practice guidelines can improve the quality of life for patients with chronic diseases. After studying the information presented here, you will be able to:
- List the three characteristics of disease management programs
- Identify five chronic diseases that are the focus of disease management programs
- Discuss respiratory care educational interventions for patients with asthma, diabetes and heart failure
