Healthcare providers are increasingly involved in teaching patients to control diabetes through skillful insulin management. Through self-blood glucose monitoring and intensive insulin therapy, patients can learn to “think like a pancreas” and manage insulin therapy to match their lifestyles. This course begins with an explanation of the actions and types of insulin and describes common problems of blood glucose management with various insulin dosage regimens. It also contains criteria for assessing patients’ readiness to take control of their insulin management. Two approaches of insulin management are covered: the prospective and retrospective.
The purpose of this course is to provide nurses, physicians, dietitians and dietary managers with the principles of insulin dosage adjustment to increase their general understanding of insulin therapy. Descriptions and examples will illustrate the prospective and retrospective approaches, rationale for carbohydrate counting and practical guidelines for using various insulins: lispro (Humalog), aspart (NovoLog), glulisine (Apidra), glargine (Lantus), detemir (Levemir), regular/standard insulin and NPH or neutral protamine Hagedorn insulin. After studying the information presented here, you will be able to:
- Name three common problems with blood glucose that occur at night
- Explain the advantages and disadvantages of the insulin pump
- Explain five general guidelines for adjusting insulin dosage
- Discuss the difference between the retrospective and the prospective approaches for adjusting insulin
- Describe three components of the prospective approach for adjusting insulin
