Summary:

Healthcare professionals have an important role in opening discussions with patients and their family members regarding advance care planning. Despite intensive efforts, most Americans do not have an Advance Directive and may face a life-threatening illness without a plan to guide their healthcare providers or relatives. End-of-life care experts stress the need for major changes in the approach to promoting advance care planning, including a focus on promoting ongoing dialogue between patients and healthcare providers. As patient advocates, clinicians are in a strong position to work with patients and other healthcare professionals to make these important changes occur.

Objectives:

The goal of this continuing education course is to provide nurses, advanced practice nurses, physicians, health educators, physical therapists, radiologic technologists, registered dietitian nutritionists, and respiratory therapists with information regarding ways to improve conversations about advance care planning such as advance directives and living wills. After studying the information presented here, you will be able to:
  • Identify barriers that patients, families and healthcare professionals face in implementing discussions regarding advance care planning such as advance directives and living wills.
  • Describe problems created by the lack of an advance directive.
  • Describe new ways that healthcare professionals can involve patients in advance care planning
