As the population of the United States ages and scientific breakthroughs advance medicine, an increase in prescription medications and consequent errors has been observed over the past several decades. Medication errors have been a highlighted safety issue targeted by The Joint Commission due to , and with an estimated 7 million patients affected annually with a subsequent cost of $21 billion, it’s no wonder why. Further, According to a seminal report published in 1999 by the Institute of Medicine, approximately about 7,000 Americans die annually due to medication errors. Medication errors occur for several reasons, including. For example, there may be a failure to communicate drug orders, illegible the handwriting on the prescription may be illegible, or the wrong selection chosen from a drop-down menu, confusion may occur between medications with similar names or sounds, the appearance of similar the packaging may appear similar between products, or there may be confusion about dosing units or weight. Medication errors usually are not due to the faulty judgment of a single person; instead, they often indicate a flawed system that has inadequate backup to detect mistakes. 

About the Course Author

Daniella M. Krantz is a Neurodiagnostic Technologist with St. Joseph Regional Medical Center. Previously, she was an Instructor to candidate pharmacy technicians with Indiana University South Bend. Dr. Kim is a residency-trained pharmacist with years of experience in academia with the Touro College of Pharmacy and in the community. She is focused on the ambulatory care aspects of pharmacy as well as mission work in foreign countries that are poor in resources.
