The goal of this interprofessional continuing education module is to provide an overview of the roles and interactions among a nurse, physical therapist, occupational therapist and speech-language pathologist as they address the complex rehabilitation and medical needs of an older adult who has experienced a post-surgical ischemic stroke.


After studying the information presented here, you will be able to:
  • Describe the distinct and interactive roles of nurses, physical therapists, occupational therapists and speech-language pathologists who work in concert to address the immediate and long-term needs of an older adult patient with complex health problems.
  • Articulate typical assessment and intervention approaches across the acute care and rehabilitation phases of treating an older adult patient who has experienced an embolic stroke.
  • Explain how the roles and functions of an interprofessional healthcare team complement one another, requiring interprofessional communication and coordination of services.

It was well done, but I would have liked to see more information regarding realistic pt transition from inpatient to home health, coordination with the inpatient and home health clinicians. Including ordering and processing DME, meds, wound care, etc.
