Course Overview

Insomnia is the most frequently occurring sleep disorder in the United States and is a source of significant distress for patients. The symptoms of insomnia are characterized by difficulty falling asleep and difficulty maintaining sleep. However, sleep problems are not the only characteristics of insomnia. Patients with insomnia suffer from impaired daytime functioning that can result in impaired cognition, poor mental performance, fatigue, and anxiety. This can lead to problems at work or school and increase the risk of motor vehicle accidents. Cognitive behavior therapy for insomnia (CBT-I) is the preferred first-line therapy for treatment of insomnia, but some patients are unwilling to participate in CBT-I, or CBT-I may be ineffective. In these cases, pharmacotherapy options may be needed. Although newer sedative-hypnotics approved for the treatment of insomnia have reduced some of the dangerous adverse effects of earlier medications, these newer agents still carry significant risks. All sedative-hypnotics can cause impaired next-day cognition and functioning, which can be dangerous for patients who drive to work early in the morning. In addition, many patients seek over-the-counter or herbal treatment options to improve sleep, even though many of these are ineffective or not recommended. It is important for healthcare professionals to evaluate patients for comorbidities that could be contributing to insomnia symptoms and to educate patients about the proper use of medications for the treatment of insomnia. Proper management of insomnia and its comorbidities can significantly improve the health and well-being of millions of patients suffering from this condition.
