According to a seminal report published in 1999 by the Institute of Medicine, approximately 7,000 Americans die annually from medication errors. Medication errors occur for several reasons. For example, there may be a failure to communicate drug orders, the handwriting on the prescription may be illegible or the wrong selection chosen from a drop-down menu, confusion may occur between drugs with similar names or sounds, the packaging may appear similar between products, or there may be confusion about the dosing units or weight. Medication errors usually are not due to the faulty judgment of a single individual; instead, they often indicate a flawed system that has inadequate backup to detect mistakes.


The goal of this medication error continuing education program is to discuss various approaches to preventing medication errors for pharmacists and pharmacy technicians. After studying the information presented here, you will be able to:
