An Underreported Infection Gone “Viral”: Trichomoniasis

Course Overview

Trichomoniasis is a sexually transmitted infection that affects more than 3.7 million people in the United States1. Since it is not a globally reportable disease, this number may be underestimated. It is associated with increased risk of HIV transmission and acquisition and should be regarded as a public health issue. Female patients may present with an unusual vaginal discharge. Described as foamy, purulent, and sometimes frothy, this discharge may be yellow-green to grey, homogenous without or without a mild fishy odor. Others may lack symptoms altogether. Highly sensitive and specific tests, such as NAAT, are recommended for diagnosis and detection of Trichomonas vaginalis. Treatment with metronidazole reduces signs and symptoms of trichomoniasis and has been shown to reduce transmission. All sex partners should also be treated with the same medication, regardless of symptoms, to reduce risk of reinfection and transmission. Current 2015 CDC guidelines also recommend retesting female patients for cure within 3 months of successful treatment. There is currently no recommendation on retesting in males. Pharmacists can play a vital role in counseling patients on safe sex practices and use of medications for the treatment of sexually transmitted disease.
